I am aware that activities of the tennis program may involve running and exertion in direct sun.
_____ PickleBall
I am aware that activities of the pickleball program may involve running and exertion in direct sun.
With full knowledge of the dangers involved, I voluntarily request that my child participate in the program.
I accept any and all risks to myself and my child in the activities and the use of the facilities and property of the Chappaquiddick Community Center, Inc. (CCC) and the M.V. Land Bank, whether or not the losses result from negligence or other action, except for intentional acts of any of its officers, directors or employees.
Waiver of Liability
I waive and release any right I, my heirs, distributes, guardians, legal representatives and assigns may have or acquire to make claim against, sue, attach the property of or prosecute the Chappaquiddick Community Center, Inc. or any of its officers, directors or employees and also the M.V. Land Bank who permits the CCC Sailing Program to be operated from their beach, for monetary damages caused by injury to my child or damage to the property of myself or my child arising from my child’s participation in the activities and use of facilities and property of the Chappaquiddick Community Center, Inc. or the use of the Land Bank beach property, whether or not injury or damage results from the negligence or other action except intentional acts of the Chappaquiddick Community Center, Inc.’s officers, directors and employees.
I have carefully read this agreement and understand its contents. I am aware that the agreement includes a waiver of liability and an assumption of risk. I sign this agreement on behalf of myself and my child, of my own free will.
